We make every effort to ensure the below information is as accurate as possible; however, due to the continuous changes among insurance carriers and their plans we highly recommend that you check with your health insurance to determine the specific details of your coverage, and to ensure that the physician you are seeing is “in-network” with your specific plan. It’s important to note that, just because we are contracted with a specific insurance, it does not mean that our providers participate in all of their products or networks. Please check with your health plan if you have questions about benefits, a specific product, or a provider network.

Please make sure to bring all of your health insurance cards to your appointment so that we may make copies of them upon check-in. Your insurance may require prior authorization before seeing one of our physicians. If this is the case, our office staff will work diligently to obtain this authorization for you. In addition, your insurance may require an office co-payment, which we ask that you pay upon check-in. If your visit is not covered by your health insurance plan, you will be asked to pay for your visit up front (for which we offer a self-pay discount). We accept cash, check, and credit cards.
