Your Appointment


To make an appointment, please contact us at 415.814.3429. Should you need to cancel or reschedule your appointment, we ask that you give us at least 24 hours notice. All of our physicians have a waiting list, and there’s always another patient who would appreciate your spot. If we know of an opening ahead of time, we can accommodate them. Thank you for your cooperation.

 

New Patient Appointment

Please arrive at least 15 minutes prior to your scheduled appointment time. Please bring your current insurance card(s). If your insurance plan requires a co-payment, you will be asked to pay it at check-in. In addition, a number of forms are required at the time of your first visit with us. For your convenience, most of these forms are available here (requires Adobe Acrobat Reader):

Please Note: Pacific Neurosurgery has implemented a policy requiring all patients to keep a credit card or debit card on file with us. This is due to an increase in the amount of deductibles and co-payments required by insurance companies, and to decrease the number of delinquent accounts. Please be assured that we will not run your credit card unless absolutely necessary.

 

Follow-Up/Post-Op Appointment

If you’ve made any changes to your insurance plan(s) since last seeing us, we ask that you bring this information with you. Co-pays will be collected when applicable. Please note that the post-op period is 90 days following your surgery, and post-op visits are included in your surgical fee. Should you see one of our physicians within that post-op period, there is no charge for that visits.
